Established in 1992, IWA Australia is a not-for-profit organisation working in the community sector providing various services like Aged Care, Settlement, Disability and Community Development to communities across SE Qld, NSW and Vic. We are looking for our next Aged Care Coordinator to join our team in our Roselands office.

As an aged care Coordinator, your main aim is to provide effective client-centred case management services and supervision of Home Support Workers, in order to ensure the delivery of flexible and individually tailored support services for a range of clients including those with high level and complex physical and/or behavioural needs.

Duties and Responsibilities:

Referrals, Intake and assessment of aged care clients.

In consultation with clients, their representatives and health professionals, develop a comprehensive support plan that meets the client needs.

Communicate regularly with clients and/or family towards achieving client goals as per care plan

In consultation with the Clinical Team, regularly review and update the care plan to ensure effective service delivery to meet the needs of clients.

Maintain accurate client records including agreements, health safety checks, TRACCS, care plan reviews etc

On-going monitoring of the quality of services received by clients, through regular client review and feedback from Home Care Employees and significant others.

Monitor clients’ HCP budget

Coordinating support services by referring to brokerage services when necessary

Essential Criteria:

Diploma of Community Services

Sound knowledge of Home Care Packages

Sound oral and written communication skills including an ability to liaise effectively with a people from diverse background

Ability to work both independently and in a collaborative team environment

Ability to assess priorities and manage competing deadlines both independently and as a member of a team

Well-developed customer relations skills and a commitment to providing quality services, and implementing continuous improvement

Drivers Licence

Ability to speak a second language (Arabic preferred)
