About the Role:
People First Healthcare is a registered aged care and NDIS provider, proudly serving communities across Vic . Passionate about delivering high-quality care you will play a crucial role in ensuring the efficient and effective allocation of support workers to clients receiving home care packages. You will collaborate closely with the care management team, support workers, and clients to deliver tailored, high-quality services that meet individual needs and preferences.
Key Responsibilities:
Rostering & Scheduling:
- Develop and maintain an efficient rostering system to match support workers with clients based on skills, availability, and location.
- Ensure all client visits are scheduled appropriately to maintain continuity of care.
- Address any scheduling conflicts or gaps to provide uninterrupted services.
Care Coordination & Client Support:
- Work with participants, families, and healthcare professionals to develop and review care plans.
- Support clients and families by educating them about available resources and advocating for their needs.
- Maintain strong relationships with clients, ensuring their preferences and concerns are addressed promptly.
Compliance & Documentation:
- Maintain accurate records of client schedules, support worker availability, and care plans.
- Ensure compliance with organisational policies, industry regulations, and documentation requirements.
- Monitor risks and implement safety measures to enhance service quality.
Continuous Improvement & Team Collaboration:
- Identify and implement improvements to rostering and care delivery processes.
- Gather feedback from clients and staff to refine service offerings.
- Communicate effectively with the care team to ensure seamless coordination and service excellence.
What You Bring to the Role:
- Tertiary qualification in Health or a related field, with relevant experience in care management.
- Strong background in home care, care management, or a healthcare setting.
- Experience in rostering or scheduling within the healthcare or aged care sector.
- Knowledge of home care packages and an understanding of aged care and disability support needs.
- Strong interpersonal and communication skills to engage effectively with diverse stakeholders.
- Sound understanding of compliance in Aged Care and NDIS, including quality standards and WHS regulations.
