We seek an experienced Registered Nurse (RN) / Case Manager to support our Newcastle and Hunter-based clients in Aged Care, Disability Support and Attendant Care (complex physical/mental). Ideally, you will have a strong mix of nursing and case management skills and enjoy working with clients in the community.

Your role will utilise your nursing and case management experience and will include the provision of nursing procedures, training and supporting field staff, managing client intake, assessments and care planning, case management, managing client budgets, as well as coordinating and monitoring all aspects of service to ensure clients' needs are met.

What is required?

AHPRA Registered Nurse

Case Management experience in supporting clients with government Home Care Packages (required), NDIS funding, and insurance clients receiving attendant care (icare)

Superior interpersonal and written/verbal communication skills

Exceptional customer service skills

Ability to build strong relationships and networks within the community

Valid Australian driver's license and access to a reliable car with comprehensive insurance

A current National Police Check

NDIS Worker Clearance

The salary package is negotiable and will include a car allowance and the opportunity to earn performance-based bonuses.
