We are seeking a compassionate and skilled Disability Support Worker to join our team in supporting a new client. In this role, you will be responsible for providing personal care assistance and intensive support to individuals with complex disabilities and challenging behavioural complexities. You will play a key role in helping all our clients navigate daily challenges and achieve their goals, particularly for those on the pathway of going home.

Responsibilities:

· Provide high-intensity support to clients with complex disabilities, including personal care, behavioural support, domestic duties and community participation.

· Assist clients in accessing community resources and participating in social and recreational activities.

· Monitor and document client progress with accuracy and facts; documenting client behaviours, and any changes in condition, reporting any concerns to the appropriate team members.

· Stay up to date with and implement individualised behaviour support plans tailored to each client's unique needs and goals.

· Collaborate with multidisciplinary teams, including therapists, healthcare professionals, and family members, to ensure coordinated care and support.

· Promote independence and self-advocacy skills among clients, empowering them to make choices and decisions about their lives.

Essential Criteria

· Certificate III or IV in Disability or equivalent qualification

· Manual Handling certification

· NDIS Worker Check

· Working with Children Check

· Police Check (no older than 12 months)

· First Aid and CPR certification

· Right to Work in Australia

· Proven experience working with individuals with complex disabilities, including behavioural support needs.

· Experience in providing personal care assistance, including support with hygiene, dressing, and mobility, and the ability to offer hands-on support in a respectful, professional manner.

· Valid driver's license and access to a reliable vehicle (preferred).
