We have a wonderful opportunity working as a Case Manager Based in Melbourne's Sunshine or Abbotsford. This is a great opportunity to take on an exciting 6month casual role with a leading and well respected NFP community care organisation with a reputation for excellence, delivering a high standard of care to the ageing and vulnerable in the community.

If you are an experienced Home Care Packages case manager, we would love to speak with you! We are looking for a dynamic, energetic case manager to support participants to remain living independently in their own homes.

As this is a short-term assignment, we need someone that can start in early April 2025, with current HCP experience that can hit the ground running.

To apply for this position, you must meet the following criteria:

• Experienced working as a Home Care Packages Case Manager- managing a caseload.
• Strong IT skills
• Experienced with care planning, budgets, and assessments.
• Positive, flexible, and dynamic personality, a self-starter who has a genuine desire to assist clients to maintain their independence and stay connected within their community.
• Ability to work independently and within a team environment.
• Able to work 5 days per week (Monday to Friday)
• Resilient with a dynamic, positive, and can-do attitude.
• Degree qualification in Nursing, Allied Health, or Social Work desirable.

We must have or be willing to get:

• Current Police Check
• Working with Childrens Check
• 3 x Covid Vaccinations
• Annual Flu Vaccination
