THE ROLE:
We are seeking an Occupational Therapist for a community based aged-care role.
As an Occupational Therapist you'll play a pivotal role in enhancing the independence, wellbeing and quality of life of your clients. As a part of this, you will conduct in-home assessments and provide clinical interventions including the recommendation of assistive technology and home modifications.
This role is based in the Metro North area (other metro regions will also be considered). This is a hybrid role working with clients in their homes, in our office and from home.
Duties will include:
- Provide expert advice, assessment and prescription of assistive technology and home modifications to CHSP eligible clients.
- Assist with coordination of a case load, planning, service delivery, and evaluation.
- Maintain appropriate clinical documentation.
- Identify and complete referrals to other services as required
THE CANDIDATE
The successful candidate will be a passionate, self-driven, and innovative Occupational Therapist.
You will be organised and flexible with a high standard of communication, problem-solving and initiative. You will be comfortable and capable to work autonomously within the community setting.
You will have sound clinical expertise, preferably within the Aged Care sector.
The candidate will possess:
- A passion for working in community based aged care.
- The ability to work with individuals, care givers, health professionals and other staff within a consultative and collaborative environment to achieve positive outcomes for clients.
- Current registration with AHPRA, NDIS Workers Screening check, WA Drivers License, and current First Aid and CPR.
