About the Role
We’re seeking a dedicated and passionate HMR Pharmacist to join our multidisciplinary team. You’ll be embedded within the practice, with direct access to our doctors and patients – enabling more meaningful, timely, and effective medication reviews.

Your responsibilities will include:

• Conducting comprehensive Home Medication Reviews (HMRs)
• Collaborating closely with GPs to optimise patient therapy and reduce medication-related harm
• Supporting medication management for high-risk patients, including those recently discharged from hospital
• Educating patients and carers to promote safe, effective medication use
• Participating in quality improvement initiatives and case conferences, as needed
  

Remuneration:

• $70–$90 per hour, based on experience and performance (employee arrangement)
• Contractor rates up to $140 per hour, depending on qualifications and workload
• Flexible engagement model – choose to join as an employee or contractor
  

What we’re looking for:

• AHPRA-registered pharmacist with current accreditation to perform HMRs (AACP or equivalent)
• Excellent communication and clinical problem-solving skills
• Ability to work independently and within a multidisciplinary team
• A passion for patient education and preventative health
