About Your Role
The Wellbeing and Health In-reach Nurse (WHIN) Coordinator CNS2 is responsible for facilitating identification of health, wellbeing and social needs and the coordination of appropriate early intervention, assessments, and referral to services for school-aged children and young people attending designated primary and secondary schools.The WHIN Coordinator CNS2 is a Registered Nurse who works in identified local schools and applies their specialist clinical nursing knowledge, experience and skill in providing complex nursing care for school aged children and young people in these schools:-
- Work with local school principals, school wellbeing teams, school counsellors and psychologists, school staff and local services to deliver the WHIN coordinator model of care service
- Build trusting relationships with students and families to identify health needs and address barriers to accessing services
- Establish a family focussed approach to connecting students and families to services
- Identify and triage the health needs of students and families
- Assist in coordination of appropriate early intervention, assessments, and referral to services and programs
- Contribute to care coordination and case management of vulnerable children, young people and families
- Support access to health promotion programs and services to improve the health and wellbeing of the school community and provide support for students and families to improve health literacy and access to services
- Work within locally agreed protocols and governance arrangements across Health and Education sectors
- Build and maintain relationships with service providers and establish effective referral pathways
- Collect and report on service events as required, maintain confidentiality and accurate health care documentation
Clinical Nurse Specialist Grade 2 (CNS2) are crucial members of the Murrumbidgee Local Health District Nursing and Midwifery Workforce. The CNS2 is responsible for the provision of high level patient centred, evidence-based clinical nursing care and is expected to embrace and lead a practice development philosophy in the delivery of that care.
The CNS2 is responsible for evaluation and supervision in the provision of complex nursing care directed towards patients within the designated area of clinical practice.
The CNS2 will demonstrate the ability to act as a professional role model, delivering a high level capacity for patient advocacy and cost benefit practice while demonstrating the ability to act as a change agent, provide effective clinical leadership and the ability to practice as an autonomous professional with the capacity to work within a multidisciplinary team environment.
