About Your Role
The Transition of Care Registered Nurse will be responsible for in-reach into Wagga Wagga Base Hospital to provide a range of assessment and therapeutic services for clients with chronic respiratory and heart failure, to support their transition home from hospital. The Registered Nurse will provide targeted education and support to patients to increase their ability to self-manage their chronic condition, and link patients with appropriate community based services. This includes ensuring patients have an appointment with their general practitioner on discharge, as well as coordinating appointments with new outpatient respiratory and heart failure services, and linking patients with pulmonary and heart failure rehabilitation, ongoing clinical nurse specialist or physiotherapy support as required.
The Registered Nurse will work alongside a newly implemented community pharmacist role, and be part of a multidisciplinary Chronic Respiratory and Heart Failure team across the Murrumbidgee Local Health District to implement evidence based practice in chronic pulmonary and cardiac rehabilitation, from diagnosis through to disease transition.
