We are excited to be hiring for a Clinical Multi Program Assessor to join our vibrant team in Coolangatta. This role will support the daily Assessment and Coordination operations for Aspire4Life's Assessment programs, specifically the Veterans Home Care (VHC) and Single Assessment System programs. As part of the position, you will be responsible for reviewing and approving plans, conducting clinical triage assessment activities, and conducting assessments.

This permanent full-time opportunity offers a dynamic mix of conducting phone-based assessments from our office and face-to-face assessments in the community.

We offer a comprehensive 2 weeks of full-time training and orientation held both virtually and face-to-face in our Coolangatta office (all expenses covered).

The role:

  • Utilise your assessment experience to conduct holistic style assessments using a client centered approach with a focus on wellness and reablement.
  • Capture vital client information and develop an individualised support plan underpinned by the clients' unique goals within their homes and over the phone.
  • Develop strategies to connect people with appropriate services and supports in their community to further empower the client with their independence.

About you:

  • Experience in assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in assessment and/or coordination of services.
  • Confident communication and interpersonal skills to engage and motivate clients, families, staff, and other stakeholders with a positive approach.
  • Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite).
  • Previous experience performing in a KPI environment to ensure good quality client outcomes.

What to bring?

  • Demonstrated experience working in the aged or community care sector.
  • Unrestricted Australian driver's licence.
  • Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker with eligibility for AASW membership.
  • Evidence of an influenza vaccination (for the 2024 Flu Season)
  • Evidence of COVID-19 Vaccinations (minimum two doses + Booster)
