About the Role
FCD Health is seeking an experienced Nurse Practitioner to join the Palmerston GP Super Clinic and Palmerston Medicare Urgent Care Clinic (PMUCC). As a key clinical team member, the Nurse Practitioner (NP) provides advanced clinical expertise, leadership, and independent decision-making in delivering high-quality, client-centered care. The NP works within a multidisciplinary team to provide comprehensive primary health care services, focusing on preventative care, chronic disease management, and urgent care interventions. The role ensures cultural competency and adherence to the National Standards for Nurse Practitioners.
This role offers the opportunity to work collaboratively with General Practitioners, nurses, allied health professionals, and administrative support staff, ensuring continuity of care for all patients.
Key Responsibilities
Clinical Practice
- Conducted comprehensive patient assessments, diagnosed, treated, and managed acute and chronic conditions within the nurse practitioner's scope of practice.
- Initiate and interpret diagnostic tests (pathology, radiology, and other investigations).
- Prescribe medications and therapeutic interventions as per AHPRA and PBS prescriber guidelines.
- Develop and implement individualised care plans in collaboration with patients, families, and multidisciplinary teams.
- Deliver culturally safe and responsive care, respecting patients’ beliefs and values.
- Provide clinical leadership, mentoring, and supervision to nursing and allied health staff.
- Participate in quality improvement activities, clinical audits, and research to enhance patient care.
Urgent Care Responsibilities
- Provide immediate assessment, triage, and treatment of patients presenting with urgent but non-life-threatening conditions.
- Collaborate with patients' usual General Practitioners (GPs), Emergency Departments, and other healthcare providers to ensure timely and appropriate referrals.
- Ensure safe prescribing practices, including access to PBS medications where applicable.
Selection Criteria
Essential:
- Registration as a Nurse Practitioner with AHPRA.
- Current Nurse Practitioner Endorsement by the Nursing and Midwifery Board of Australia (NMBA).
- Minimum of five (5) years post-graduate experience in nursing, with at least two (2) years in an advanced clinical role.
- Proven ability to work autonomously in clinical decision-making, assessment, and patient care management, preferably in Primary or Urgent Care.
- Demonstrated understanding and experience in primary health care, chronic disease management, and preventative care.
- High-level communication and interpersonal skills, committed to culturally competent care, particularly for Aboriginal and Torres Strait Islander communities.
- Experience in clinical leadership, mentoring, and policy development.
- Commitment to quality improvement, research, and evidence-based practice.
- Proficiency in electronic health record systems and digital health technologies.
- Current Prescriber Number for Medicare Benefits Schedule (MBS) and Pharmaceutical Benefits Scheme (PBS).
- Current Working with Children Clearance (Ochre Card).
- CPR Certificate (with an obligation to maintain certification).
Desirable:
- Postgraduate qualification in Primary Health Care, Indigenous Health, or Chronic Disease Management.
- Experience working in rural, remote, or Aboriginal Community Controlled Health Organisations.
- Knowledge of Medicare Benefits Schedule (MBS) billing and funding models for Nurse Practitioners.
