We are looking for a Clinical Coordinator to join our team in Brisbane office. This is a maternity leave position and will be a 12-month contract with a chance of extension. The Clinical Coordinator provides clinical assessments to IWA clients and clinical leadership to Coordinators – Case Management, Coordinators - Rostering, and support staff. The role ensures the continued success of home care programs and overall team coordination at IWA Australia. The clinical coordinator will be reporting to Operations Manager.

As a Clinical Coordinator at IWA Australia, your main tasks and responsibilities will be:

Referrals, Assessment and Re-assessment

• Assist in the submission of referrals to access services or higher levels packages.
• In conjunction with clients, significant others and health professionals, conduct clinical assessments of all IWA client admissions and reviews, meeting program-specific requirements
• Liaise with the Coordination Team to make clinical recommendations of supports

Support Plan Development & Service Delivery

• In consultation with clients, their representatives and health professionals, develop a comprehensive support plan that meets the clients’ clinical and holistic needs
• Regularly review and update the care plan to ensure effective service delivery and compliance with quality standards
• Ensure that all clinical documentation is completed accurately and in a timely manner in accordance with IWAA’s policies and procedures.
• As a member of the clinical team, make decisions regarding questionable items posed within the Inclusions/Exclusions Framework of the Home Care Package Program
• Record and monitor significant health conditions.
• Collaborate and liaise with allied health services and medical practitioners as needed.

Quality Assurance & Continuous Improvement

• Record and follow up on clinical issue and incidents
• Assist in the development and review of clinical policies and procedures
• Record and follow up on incidents / complaints as required.
• Assist in maintaining IWAA’s clinical governance framework and compliance

Reporting, Education & Training

• Foster teamwork and communication with staff, clients, and allied health professionals.
• Provide ongoing clinical leadership, advice and guidance to Coordinators – Case Management, Coordinators and support staff.
• Assist in developing a training calendar for support staff.
• Record and collate clinical data trends
• Prepare & submit monthly clinical team report to the Quality Advisory Committee

Key Selection Criteria:

• A qualification/experience in a relevant industry area

• Well-developed oral and written communication skills including an ability to liaise effectively with a people from diverse backgrounds
• Ability to work both independently and in a collaborative team environment
• Ability to assess priorities and manage competing deadlines both independently and as a member of a team
• Demonstrated high level literacy, numeracy, accuracy and attention to detail skills.
• Excellent interpersonal and communication skills, including the ability to consult, negotiate and liaise effectively with a diverse range of people
