The Medical and Critical Care team at the Mackay Base Hospital is seeking applications for a Nurse Navigator (Stroke Coordinator) to join their team on a permanent basis.

Your opportunity
The purpose of this role is to demonstrate accountability at an advanced practice level for managing and coordinating ongoing care plans for consumers and their families within Mackay Base Hospital and for the provision of nursing leadership specialist advice, consultation, and education to inter-professional team members as relevant to the specific context of the role.
The key principles for the Nurse Navigator role include:
- Coordinating patient-centred care across the patient journey.
- Creating partnerships, for example, providing a central point of communication, engagement and coordination for all stakeholders involved in the patient's care.
- Improving patient outcomes through the delivery of evidence-based care.
- Facilitating systems improvement – providing a leadership role within the organisation and act as an agent of change.
About the Team
The Nurse Navigator function is a fundamental part of the nursing role. It aims to facilitate the patient journey in what has become an increasingly complex health system. This facilitation will be achieved by reducing fragmentation, mitigating barriers, educating, empowering, and coordinating patient care. These roles are intended to transcend across multiple specialties, support nurse led service models and augment existing models of care within a facility or clinical area.
The role of the Nurse Navigator (Stroke Coordinator) is to coordinate the care and on-going management of patients who have experienced stroke.
