Daily Tasks:
- Deliver person centered supports to Early Childhood participants from a culturally diverse background.
- Support participants with a disability in developing their skills throughout their Home and Living supports program.
- Proactively work in a Group Home setting assisting participants to achieve their goals in accordance with their values and beliefs.
- Support participants to develop their skills and increase independence with their daily activities and community engagement preferences.
- Monitor progress, complete accurate case notes and reports in line with participant support plan requirements.
- Assess the participants needs and conduct a periodic review of support plans in collaboration with stakeholders.
- Evaluate risks, communicate with stakeholders and implement effective mitigation strategies promoting safe practices and dignity of risk.
- Conduct periodic assessments of participant progress and level of support needs in preparation for funded plan reviews.
- Uphold mandatory reporting and referral protocols in safeguarding participants rights, health and wellbeing.
- Support participants with Positive Behaviour Support strategies promoting a safe and supported environment within the household and community.
A few more things which will assist you in delivering the above tasks:
- Cert III, IV in Disability and Individual Support or an equivalent of 2 years' experience required.
- Experience with mental health, complex and challenging behaviours, and high physical support needs will be highly regarded.
- A current Australian Driver's Licence and vehicle with comprehensive car insurance.
- Working With Children Check (or willingness to obtain).
- National Police Check (or willingness to obtain).
- Current First Aid and CPR certificate (or willingness to obtain).
- Manual Handling training and certificate.
- The ability to develop positive relationships with our clients and to support their individual care plans.
- Excellent verbal and written communication skills.

Hours of Work:
Hours are flexible in accordance with NDIS participants' plans, however if you are a fantastic disabilities services officer, we will certainly accommodate flexible work hours.

Remuneration: $74,000 excluding superannuation
