About the role
The Assessment Officer plays a crucial role in the Commonwealth Home Support Programme (CHSP), the Home and Community Care Program for Younger People (HACC-PYP) and Home Care Packages (HCP).
This position involves conducting comprehensive assessments for clients who have been deemed eligible by My Aged Care.
The Assessment Officer evaluates the needs, capabilities and preferences of clients, developing personalised care plans that promote independence, safety and quality of life.
The Assessment Officer works closely with families and various service providers to ensure coordinated and effective delivery of care.
The role requires a client-centred approach, excellent communication skills and an understanding of the CHSP, HACC-PYP and HCP guidelines.
By regularly reviewing and updating care plans, the Assessment Officer ensures that services remain responsive to the evolving needs of clients, helping them to maintain their independence and well-being.
About You:
- Experience: Previous experience in aged care, community services or a similar role involving client assessments and care planning is beneficial but not essential as training will be provided
- Knowledge: Understanding of CHSP, HACC-PYP and My Aged Care frameworks, policies and procedures is beneficial but not essential as training will be provided
- Skills: Excellent interpersonal skills and communication skills, strong organisational and time management abilities and IT proficiency.
