Our Case Managers are the heart of this mission, empowering elderly Australians by connecting them to tailored services and championing their wellbeing.
Your Role
As a Case Manager, you will be at the forefront of delivering essential home care services. You will work closely with clients, their families, and other health professionals to assess needs, develop personalised care plans, and ensure the best possible support is provided.
Your role will involve empowering clients to make informed choices regarding their Home Care Packages, optimising their services to enhance independence and wellbeing. You will collaborate with internal teams and external partners to create a seamless experience for those you serve.
About You
You will be someone who is passionate about health and wellbeing, has a knack for building relationships, and is motivated to contribute to a supportive community.
You will also have:
- Certificate III Qualifications (or higher) in Case Management, Aged Care, or related fields.
- Experience in case management across Disability, Aged Care or Youth sectors.
- Strong communication skills with a commitment to person-centred outcomes.
- A driver’s license, along with the confidence to work autonomously and as part of a team.
