About the role

We are seeking an Experienced General Practice RN to join our team at The Banyans Medical Center in Bowen Hills, QLD. As an experienced General Practice RN, you will play a vital part in delivering high-quality healthcare to our diverse patient community.

What you'll be doing

• Managing the Treatment Room
• Perform ECG, spirometry, immunisations, wound care, assist with procedures, sterilising, recalls and reminders, triage
• Assess, develop, implement, and evaluate individualised care plans for patients with complex health needs
• Collaborate closely with General Practitioners and multidisciplinary teams to ensure coordinated patient care
• Provide guidance and support to patients and their families in navigating the healthcare system
• Manage and coordinate GP Management Plans (GPMPs), Team Care Arrangements (TCAs), and Coordinated Veterans' Care (CVC) programs
• Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with relevant legislation
• Participate in ongoing professional development and training to maintain current knowledge of best practices and industry standards

About You:

• Registered Nurse (RN) with current AHPRA registration
• Experience with Best Practice software highly regarded
• 2-3 years+ General Practice nursing experience highly regarded
• Experience with veteran healthcare and CVC plans highly regarded
• Demonstrated proficiency in developing and managing care plans, including GPMPs, TCAs, and CVCs
• Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team
• Strong organisational and time-management skills, with the ability to prioritise tasks and manage competing demands
• A commitment to delivering patient-centred care and a passion for improving patient outcomes
• Immunisation Program Nurses highly regarded (or willingness to obtain)
