Locum Social Worker | Regional Victoria

Are you an experienced and compassionate Social Worker looking for a meaningful role within a healthcare setting? We are seeking a dedicated Locum Social Worker to join a leading public hospital, providing high-quality support to inpatients and outpatients across various departments.

About the Role

As a Social Worker, you will be responsible for:

• Discharge planning of patients who are returning to aged care facilities or to their homes, ensuring they have the support and recources require to aid in their recovery
• Coordinating referrals for ongoing support.
• Collaborating with medical teams to develop holistic patient care plans.
• Leading family conferences to discuss future care options.
• Acting as a resource for community support services and mental health needs.
• Working with families and stakeholders

What We're Looking For

• A tertiary degree in Social Work
• Experience working as a Social Worker with an older adults caseload
• Experience in discharge planning and an understanding of aged care processes
• Strong communication and interpersonal skills.
• Ability to work effectively within a multidisciplinary team.
• A commitment to high-quality patient care and continuous improvement.
• Current police check, working with children check, and driver's license.
• AASW registration desirable
