Our new Youth Disability Support Services focus on services to empower young people with disability as they transition to adulthood.

This service will provide the support young people need to build essential life skills, boost their confidence and strengthen their community connections.

Make an Impact

We have an exciting opportunity for a Youth Disability Case Manager to join our team in Calamvale and support young people engaged with the child protection system who have disabilities, in 24/7 Supported Independent Living. Balancing youth work and disability care, you will create safe, nurturing environments that empower young people to achieve their goals.

Using person-centred, trauma-informed practices, you will foster emotional, social, and cultural wellbeing. You will develop and oversee Individualised Support Plans, ensuring young people remain at the core. Leading a small team, you will guide staff, coordinate with stakeholders, and promote team cohesion. With a passion for positive change, you will help young people build independence as they transition to adulthood.

Other key duties will include:

  • Develop and review Individualised Support Plans (ISPs) for young people, empowering them to lead their lives and achieve their goals.
  • Identify and manage risks related to each young person’s support.
  • Collaborate with key stakeholders, agencies, and NDIS coordinators to align services with goals.
  • Provide advocacy across health, education, employment, and community involvement.
  • Monitor service delivery and encourage participation in meaningful activities and pursuits.
  • Train, mentor, and model person-centred practices for staff.
  • Maintain accurate records, case notes, and reports while ensuring privacy.
  • Create a safe, supportive, and homely environment, promoting structure and routine.
  • Support young people experiencing AOD issues and provide appropriate interventions.
  • Coordinate with disability services to enhance capacity building and daily support.

What you can bring to the team

Ability to foster a performance-driven, collaborative culture, providing clear direction aligned with organisational goals. With strong leadership, you will proactively support staff through training, guidance, and upskilling, while promoting effective teamwork and communication.

We also require:

  • Bachelor’s degree in human services or a related discipline.
  • Minimum 2 years’ human services experience in Case Management.
  • Previous experience working with young people within a disability context with demonstrated experience of trauma informed practice will be advantageous.
  • Excellent interpersonal skills, and the ability to communicate effectively to a diverse range of people.
  • Proficient computer skills and experience using Word, Excel, and Outlook.
  • Hold or be willing to obtain a Working with Children Check (Blue Card), NDIS Worker Screening Check (Yellow Card), Child Safety and Personal History Screening Check (LCS2), First Aid, CPR, National Criminal History Check (Fit2Work), and an open Driver’s License with comprehensive insurance on your vehicle.
