The Registered Nurse (RN) in NDIS care is responsible for delivering high-quality, person-centred nursing care to individuals with disabilities. This role involves conducting comprehensive health assessments, developing individualised care plans, coordinating multidisciplinary healthcare services, and supporting participants to achieve their health and personal goals. The RN ensures compliance with NDIS guidelines and best practices while maintaining the dignity and independence of participants.
Key Responsibilities
Clinical Care & Health Monitoring
- Conduct comprehensive health assessments to determine individual participant needs.
- Monitor participants’ health conditions and intervene as necessary to prevent deterioration.
- Implement and administer personalised care plans based on clinical assessments and participant requirements.
- Provide specialised wound care, medication management, and administration.
- Support diabetic management plans, seizure monitoring, and tracheostomy care.
- Assist with dysphagia, continence, and podiatry support.
- Manage healthcare routines around the participant’s lifestyle to ensure maximum comfort and independence.
- Participate in on call rostering
- Conduct mental health assessments for the participants.
Service Coordination & Multidisciplinary Collaboration
- Work closely with allied health professionals, including speech pathologists, occupational therapists, and dietitians, to ensure holistic care delivery.
- Coordinate participant services with external healthcare providers and ensure seamless integration of care.
- Attend case conferences, service meetings, and care plan reviews to advocate for participant needs.
- Support the implementation of therapeutic service delivery frameworks in collaboration with the Team Leaders, Manager Service Delivery and all internal and external stakeholders.
Compliance & Professional Standards
- Ensure adherence to NDIS Quality and Safeguards Commission standards.
- Maintain accurate clinical documentation and update care plans as required.
- Comply with all legal, ethical, and regulatory healthcare standards, including AHPRA guidelines.
- Provide training and education to support workers and carers regarding best practices in disability care.
- Participate in continuous professional development to stay updated on industry trends and best practices.
Client & Family Engagement
- Facilitate the onboarding of new clients and their families, ensuring a smooth transition into services.
- Uphold a client-centred approach that respects participants’ choices, preferences, and autonomy.
- Act as an advocate for participants, both internally and externally, to ensure their healthcare and social inclusion needs are met.
- Promote positive community engagement and independence for individuals receiving NDIS support.
Qualifications & Experience
Essential:
- Registered Nurse qualification and AHPRA registration.
- 5+ years of experience in disability nursing or community healthcare.
- Strong knowledge of NDIS regulations and compliance requirements.
- Excellent assessment, care planning, and clinical decision-making skills.
- Proficiency in electronic health records and care documentation.
- Experience in coordinating multidisciplinary healthcare teams.
- Certification in wound care, diabetes management, or similar specialisations.
- Training in person-centred active support and therapeutic care models.
- Full driving licence and access to registered vehicle.
