We have an exciting opportunity for an Aged Care Assessor to join our team in Northern Sydney. In this community-based role, you will work closely with clients in their homes, utilising your clinical expertise to conduct assessments to support individuals in navigating their aged care journey. This role will also conduct Residential Aged Care Funding Assessment.
We offer both part-time and full-time permanent contracts, working Monday to Friday-no weekend or shift work required.
To ensure you feel confident and supported in your role, we provide a comprehensive two-week training and orientation program, first week is in-person at our Coolangatta office. All expenses are covered, and the next orientation intake begins March 10, 2025.

The role:
- Working independently within your region, utilising your assessment experience to conduct holistic style assessments with a client centred approach focusing on wellness and reablement
- Capturing individual client information and working collaboratively to develop their support plan and unique goals
- Supporting older Australians to continue living independently, by considering supports such as assistive technology, social support services, and community care.
- Utilising the integrated assessment tool with My Aged Care
About you:
- Experience in assessment and goal orientated support planning in aged, community, health or social services
- Confident communicator with a positive approach when collaborating with clients, families, staff, and other stakeholders
- Local knowledge and the ability to network with stakeholders within your region
- Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite)
- Previous experience working with KPI's to ensure high quality client outcomes
- Ability to manage your time effectively in an autonomous working environment
What to bring?
- Minimum Certificate III in Aged Care, Community Services, or similar and/or 3 years' experience in working in aged care /community services
- Unrestricted Australian Drivers Licence
- Evidence of an influenza vaccination (for the 2024 Flu Season)
- Evidence of COVID-19 Vaccinations (minimum two doses + Booster)
