Aged Care Placement Services was formed in 2012 to address the frustration and confusion that comes with navigating the aged care industry. We help older Australians find the right care: the best providers, services and residential solutions based on their individual needs.
We are specialists within the aged care industry, and work specifically with vulnerable seniors who need intensive support to access care, allied health professionals and support, housing and other services that will assist them live safely in their home and age with dignity and respect. Our clients have little to no support from carers or family. In a lot of cases have difficulty communicating because of language or literacy problems and find it difficult to understand information and make decisions.
Our mission is to ensure Australia’s ageing population are aware of their rights and available options, so that they can make informed decisions on how they would like to proceed in this new chapter of their lives.
About you
The Occupational Therapist role is a pivotal role in ensuring our clients receive outstanding customer service tailored to their unique needs. This individual plays a vital part in the team dedicated to the care and comfort of our clients.
Key Responsibilities
Conduct comprehensive assessments to determine clients' functional abilities, mobility, and home environment, identifying areas where interventions are required.
Develop individualised occupational therapy care plans that address clients' needs in mobility, daily living activities, and the safe use of equipment.
Provide therapy to help clients regain or maintain independence in daily activities such as bathing, dressing, cooking, and managing medications.
Recommend home modifications or assistive devices to improve safety and functionality in the client's living environment.
Offer guidance and support to clients and their families regarding strategies to manage age-related challenges, enhance independence, and adapt to changing health needs.
Work with care providers, healthcare professionals, and social workers to ensure an integrated and holistic approach to client care.
Maintain accurate and timely client records and progress notes in compliance with organisational and regulatory requirements.
Raise awareness of clients' rights and available care options, empowering them to make well-informed decisions about their care.
Stay up to date with best practices in aged care, and participate in training and professional development activities.
Essential Criteria
Proven experience as an Occupational Therapist, ideally within the aged care or disability sectors.
Strong understanding of comprehensive assessments for aged care clients.
Strong understanding of comprehensive assessments for clients seeking to access NDIS.
Strong understanding of comprehensive assessments for home modifications.
Bachelor’s degree in Occupational Therapy or equivalent.
Current registration with AHPRA as an Occupational Therapist.
Experience in providing assessments and developing care plans for elderly clients with diverse needs.
Strong understanding of aged care legislation, policies, and best practices.
Strong problem-solving and decision-making skills with the ability to prioritise and manage a caseload.
Proficiency in using electronic medical records and documentation tools.
Compassionate, patient, and committed to improving the quality of life for elderly individuals.
Strong interpersonal skills with the ability to work autonomously and as part of a team.
Excellent time management, organisational skills, and attention to detail.
