As a Community Disability Support Worker, you’ll be right there beside people with complex needs, supporting them to build independence, grow their confidence, and chase goals that matter most to them. One day you might be shooting hoops or dancing to your favourite playlist, the next you’re exploring a museum, hitting the beach, or go-karting. You’ll be helping people try new things, grow in confidence and hit their goals.
Here’s what a typical day might look like (spoiler: it’s rarely “typical”):
Mentoring and role-modelling in real-world situations in response to client needs
Assisting clients with elements of their daily living as detailed in support & care plans.
Supporting clients to explore & engage in their interests and hobbies to enhance social & economic participation
Increasing client’s independence by supporting their decision making & problem solving
Supporting community clients with person centred supports working towards their individual NDIS goals.
Transporting clients to activities, appointments, or adventures as required using your own vehicle.
This is a part-time role, across East Metro, South Metro or both. Offering up to 70 hours per fortnight, depending on your availability. We’re looking for people who can be available at least 3 days a week (Monday to Friday) in one or more of the timeslots listed below. You may be rostered to work with multiple clients in a day, allowing for breaks & paid travel.
Shift options 1: 08:00 – 14:00
Shift options 2: 10:00 – 18:00
Shift options 3: 14:00 – 21:00
👉🏼 The need-to-have…
Here at Rise, we nurture potential, but we do need you to have a few tricks up your sleeve including the following:
You’ve studied: A qualification in Human Services like Disability, Aged Care, or Community Services.
You’ve walked the walk: You’ve got hands-on experience supporting people with disabilities and complex needs—and you’re great at it.
You’re a goal-getter: You know how to develop, deliver & assess plans that help clients kick goals.
You’re independent but never alone: Working solo out in the community doesn’t faze you—you’ve got initiative, but you also know when to check in.
You keep calm: Training in de-escalation, trauma-informed practice, or MAYBO is a bonus—but even without it, you know how to stay cool and compassionate under pressure.
You understand uniqueness: Experience working with people with ASD or mental health conditions is a big plus.
You’ve got the time: You're available at least 3 days a week and happy to work weekday shifts between 8am and 9pm.
You can drive: This position requires a driving licence and reliable vehicle to use for work. Depending on a person’s support needs, you may be required to travel across Perth Metro and drive a modified van. Applicants’ flexibility and willingness to travel is highly regarded.

Appointments to this position require a NDIS worker screening check. Costs for new checks will be covered and/or reimbursed by Rise.
