Position Overview:
We're looking for a passionate Clinical Care Coordinator to join our PHN programs team. In this client-focused role, you’ll be the key link between clients, psychiatrists, GPs, and support workers. From intake to follow-ups, your role is vital in ensuring smooth coordination, accurate data entry, and positive client outcomes.

Key Responsibilities:

Conduct client intake and follow-up appointments, tracking progress with outcome measures

Facilitate referrals to mental health professionals and support services

Maintain accurate client records and demographic data

Manage communication between clients, psychiatrists, and other stakeholders

Support continuous improvement of program delivery and client experience

Commit to ongoing learning and development

What We’re Looking For:

Strong organisational skills and the ability to juggle multiple tasks

Excellent written and verbal communication skills

Empathetic, adaptable, and committed to high-quality client service

Meticulous attention to detail

Education & Experience:

HSC, diploma or degree in psychology, counselling, or social work preferred

6–12 months experience in customer service or mental health clinics

Familiarity with the Australian healthcare system is highly desirable

Experience in contact or medical centres is a plus

A current Australian driver’s licence and reliable vehicle are essential. Some travel will be required.
